Selected Information from the Full Matrix


The ADRC-TAE researched several Web sites associated with single point-of-entry systems and compiled a matrix that describes the site content, emphasizing resource databases, and other administrative information that may be helpful for grantees as they create or enhance existing Web sites associated with the ADRC program. Information for this matrix was obtained from the Web sites and through telephone interviews with the lead contacts for each site.

California (San Diego) Network of Care

Four San Diego County, up-to-date, online databases – 1) seniors and people with disabilities; 2) mental health; 3) children and families, and; and 4) kids. The Web site also includes a user-friendly Assistive Devices Database of more than 19,000 assistive tools that you can search by function, manufacturer and brand name.
Georgia AgeWise Connection

Connection online database contains a snapshot of the most requested services and providers in Atlanta Region and the State of Georgia. Search results include basic info: provider names, addresses and tel. Users are then directed to call the AgeWise Connection phone line for more in-depth information. AgeWise Connection Plus software and database is for professionals who work with older adults; providers pay a subscription fee and have access to over 4,000 providers and 15,000 aging services.

Universally-designed information system listing a broad spectrum of over 144,000 statewide community resources. Offers customization options for users to retain records and access online applications for Medical Assistance and Financial Assistance/Food Stamp benefits.
Massachusetts Senior Connection

Online statewide database, “Guide to Elder Services”, allows users to search 796 agencies and nearly 1,005 program listings and descriptions designed for seniors and caregivers. Database is aimed at seniors and caregivers.

Comprehensive online statewide database inclusive of community services broader than long-term care. Informationis organized using the INFO-LINE taxonomy. Database aimed at older adults, individuals with disabilities of all ages, caregivers, and providers.

Database targeted at older adults and caregivers provides a statewide listing of services. The site also provides additional educational resources. Users have the option of registering which provides the ability to create a personalized folder, ask questions of an expert, and receive e-mail updates on the site.
