Welcome

 

Welcome to the ADRC Readiness Assessment.  The Lewin Group designed this assessment to assist organizations in evaluating their readiness to perform key functions of Aging and Disability Resource Centers (ADRCs).  The assessment is:

     

  • Easy to use--click on the responses that most accurately describe the present status of your organization or agency
  •  

  • Confidential--all information is password protected and available only to the user
  •  

  • Informative--assesses organizational readiness in seven functional areas of ADRCs and many of the questions  include links to additional information or resources about those items
  •  

  • Responsive--results are evaluated and immediate feedback is provided
  •  

  • Convenient--the survey takes about 30 minutes to complete and, if necessary, you can save your work and come back later.

 

If you are unable to complete the online version at this time, you may download the Print and Score version of the Readiness Assessment in PDF. The Print and Score version has similar content to the online tool but will not offer you the same level of customized feedback.

 

Three Easy Steps

 

You Can Complete the Survey and Obtain Your ADRC Readiness Profile in Three Easy Steps

 

Step I --  Provide basic background information about your organization and create a unique password. This will ensure the confidentiality of your responses and will enable you to save your survey and return to it in the future, if needed.  The password you create will also be required to access your ADRC Readiness Assessment Results which will appear after you have submitted the completed survey for analysis.

 

Step II -- Complete the ADRC Readiness Assessment Survey.  The survey asks you to provide information about your organization in seven key functional areas of ADRCs: 

 

1. Organization and Governance

2. Personnel Management and Training

3. Service Delivery and Operations

4. Outreach and Marketing

5. Information Technology and Management Information Systems (IT/MIS) Capacity and Support

6. Partnerships

7. Evaluation and Monitoring

 

Respondents are asked to answer a series of multiple choice questions in each area indicating a response that most closely matches the current capacity of their organization. The questions in the ADRC Readiness Assessment Survey are based on criteria found to be most closely associated with fully-functional ADRCs based on the experiences of the programs over the past five years. 

 

Step III— Review your responses and submit your completed survey for analysis.

 

After the completed survey is submitted, your ADRC Readiness Assessment Results will appear on the screen. These Results will detail your organization's level of readiness in the main areas covered in the survey and offer links to resources that may help your organization build capacity in these areas. You should print your Readiness Assessment Results, but they will also be saved in the system and you may access them again online by using the unique username and password you create at the beginning (in Step I above).
